Introduction

Several reports in recent years have raised concerns about the mental health of young people, suggesting rising rates of emotional problems and self-harm. Self-harm is most common in people under 25 and although suicide rates at this age are comparatively low, the latest figures from 2015 show a small rise. The National Strategy (Preventing suicide inEngland: Third progress report 2017) is strengthening its focus on men, self-harm and support for bereaved families.

Given the direct link to suicide and increasing concerns about self-harm raised by professionals who work with children and young people, it is timely that we increase our efforts to address this issue. The latest data also shows a higher prevalence of hospital admissions of girls under 17 for self-harm.

Self-harm, including attempted suicide, is the single biggest indicator of suicide risk. The UK has high rates of self-harm resulting in over 200,000 hospital attendances per year in England. Approximately 50 per cent of people who have died by suicide have a history of self-harm, and in many cases there has been an episode of self-harm shortly before someone takes their own life.

Complex factors and events may contribute to the death by suicide of children and young people. A recent study by NCISH found academic pressures, bereavement, bullying, alcohol or drug misuse and childhood abuse in many cases. More than half of those who died had a history of self-harm and 27 per cent had expressed suicidal thoughts in the week prior to death.

Questions for Tweetchat Audience

Mostly healthcare professionals

What do you think is behind the rise in rates of self-harm in young people and particularly girls in their teens, over the last 30 years?

Small things can make big differences to young people who are self-harming or suicidal.... What small things can we do? And do we do enough of this?

Do you think that we have enough compassion for, and listen properly, to young people in distress?

Why do you think that young people who are self-harming or even suicidal all too often report that adults they see are not really listening, not taking them seriously, and not being compassionate with them?

Most young people who are self-harming or even some who are suicidal don't see mental health specialists. What support is there out there for any non-specialist adult who comes across a child or young person who is self-harming or suicidal?

What could we do to provide better information and support to young people and their families who have been bereaved by suicide?

I was a Samaritan for about 10 years, off and on. I learned early on that time is one of the most welcome things that we can give. No interventions are made, no advice given, just listening. Sadly that's not always the case where I work, and the urge to "fix" things is great too. We can't always do that, we have to be realistic about the options available, and we have to encourage people to keep coming back when they need support. It is interesting to see the information and advice in this tweet chat. I will be investigating Papyrus, and have already downloaded Hub of Hope to look at later.

@WeNurses @WeAHPs Social media has a lot to answer for. Only happy lives are portrayed, people feel like everyone’s having a great life apart from them (on a bad day) but also helps mask people feeling down, it’s easy to portray that you’re happy when you aren’t & actually need some support #weMDT

@AgencyNurse @Sean_C_Ralph @WeNurses For sure but we do know from research that #LGBTQ individuals have higher rates of risk taking behaviour, worse experiences with #NHS staff than Joe public so I do think we could improve this - agree not the solution but an important improvement #WeMDT

@WeNurses The feelings for the CYP can remain difficult even after self harm. Equally hcp & parents are human and that no matter how much experience they have in supporting people, it's still ok to not feel confident or to struggle. Every person and every time is different #WeMDT

@knitknitqueen @AgencyNurse @WeNurses Apologies all, I posted what I thought was the correct link for ASIST by Living Works. I think there are various organisations who are trained to deliver the training, and I've linked to one of them instead. Google again for ASIST London? It's definitely free in my area #WeMDT

@NatashaSmasha @AgencyNurse It would be incredibly useful to have school or MH nurses in all academic establishments. In my opinion, most Universities have a huge need to increase their support to students in general. Educational establishments are an important link in prevention and support #WeMDT
