End of life care is an aspect of care that will be encountered in most clinical settings.  Around half a million people die each year in the UK, and many of these deaths are expected.

As well as experiencing difficult symptoms, many people suffer from psychological, social and spiritual concerns.  During the last year of a person's life, they are likely to meet a range of health and social care professionals for a range of different reasons.  It can be a confusing and turbulent time for the person and those important to them.  Carefully coordinated multi-professional care is essential, regardless of where the person may be.  However we know that care is not always as good as it might be, for many different reasons.

One of the goals of @WeEOLC is to encourage people from different professions to get involved in discussing end of life care.

We invite any health or social care professional to participate in this TweetChat, in which you will meet Dan (A fictional character), who has just been diagnosed with Motor Neurone Disease.  The accompanying reading is optional but might help you understand some of the challenges facing people with life-limiting illnesses, as well as help you to think about some of the innovations that can help to support people like Dan.  

Dan's journey will unfold throughout the chat via tweeted updates which will question we'll invite you all to respond to given your interest, experience and area of expertise.
