Celebrate Me: Evidencing the impact and securing the future of Learning Disability Nursing

Introduction

This is the final in a series of three Tweet Chats, guest hosted by FoNS. FoNS has developed a project, with support from NHS England and Health Education England (London), to enable the voice of learning disability nurses delivering frontline services to come to the fore and influence the future at at a time when the role of learning disability nursing in the profession faces significant challenge.

FoNS believes that understanding the perspective and experience of learning disability nurses in practice must inform the future preparation, work and ongoing development of learning disability nurses. This would add value to existing activity by evidencing the important role learning disability nurses have across health and social care; not just for those people with a learning disability but across the population.

FoNS’ aspiration for this project is to generate a new narrative for learning disability nursing and so, in order to reach out to nurses as widely as possible the three chats aim to capture experiences from across primary, secondary and tertiary health and social care.

Background to ‘Celebrate Me’

The review of learning disability nursing undertaken in 2011-12 and subsequent report ‘Strengthening the Commitment’, has offered recommendations for modernising learning disability nursing roles and practice. In 2015, the UK Strengthening theCommitment Steering Group published their report celebrating the achievements towards realising these recommendations. They also outlined an ongoing agenda to further strengthen the role of and leadership amongst learning disability nurses, regulation and quality. 

In line with Government policy and best professional practice, the delivery of learning disability services need to respond to the challenges across health and social care and change to meet the needs of a growing group of people using services. Learning disability nurses also need to identify where they can make their most telling contribution to care delivery across primary, secondary and tertiary settings. To enable this, it will also be necessary describe the educational and training interventions required to address education and development needs of the current workforce to support retention. However, this is also against a backdrop of serious concerns about the reduction in applicants for learning disability nursing graduate programmes.

‘Celebrate Me’ aims to challenge the current perceptions of learning disability nurses and nursing, by working with policymakers, educators, employers and people using services to demonstrate the unique and important contribution that learning disability nurses make across all care sectors.

The site is also a space where you can post additional comments and stories from practice. As the project progresses, details will be posted of other ways nurses, other professionals and people using services can engage in the project. A final report from the work will be published in March 2019.

Tweet Chat

At this final tweet chat on the 17th July 2018, at 2030 hours BST, the focus will be on learning disability nursing within the tertiary care sector. In accordance with the NICE glossary of definitions this includes care for people needing complex and specialist treatments. This may include specialist forensic provision, secure/inpatient services, end of life care and management of long term conditions. People may be referred for tertiary care from either primary or secondary care.

4-What support do learning disability nurses working at the frontline of services in tertiary care need to strengthen their role?

5-Is the current educational provision for learning disability nursing fit for purpose, what needs to change to enhance this and how would you articulate the importance of sustaining programmes to universities etc?

6-Finally, what would your ‘elevator pitch’ to government and policy makers be about the importance of learning disability nursing?

WeLDNurses Post Chat Blog

Introduction

On Tuesday 17th July, the WeLDNurses community was pleased to support the third and final #CelebrateMe chat hosted by the Foundation of Nursing Studies (FoNS), focussing on an adapted version of NICE’s definition of tertiary care (2018) for this context, interpreted as forensic and secure services, end of life care and management of long term conditions.

Similarly to previous #CelebrateMe chats, two data sources are considered in this blog, the demographic information shared in the Polls set up and interpreted by Ross Wyatt, and the Chat transcript themes by Daniel Marsden. These data sources have been harnessed utilising the hosts – FoNS- expert facilitation skills, eliciting some important themes for consideration and essential evidence that has had the whole WeLDNurses team reflect on our profession, the ever-present narratives, assumptions, tropes, myths and received wisdom, and the possible actions for consideration.

Poll Summary

Poll 1 – Participants role

Similarly to the previous chats, the majority of the respondents were Learning Disability Nurses or Student Nurses.  In addition to the responses provided we had respondents who were working in education/academic rolls, leadership roles some support work rolls and some who worked in art therapies.  It remains encouraging that we had a good percentage of people from other health professions sharing their views and whilst the percentage of family carers was low, it is still fantastic to have their views represented.

Poll 2 – Geographical Location

This chat, similar to the previous, had a good spread of representatives from across the areas and on with the exception of Ireland, we didn’t get any noted respondents in the first chat from this area either, this could be that people haven’t voted however as the respondent number does not match the participants from the chat. 

Poll 3 – Professional Location

A consistent result in all three chats is that of the majority of respondents identifying as part of the NHS.  There was a small percentage of representation from other statutory services and 3rdsector/charities with a good percentage from education.  It would be good to have more representation from other areas of working practice but is encouraging that there is some variation in the participants.  Again, this poll didn’t attract all participants from the night so there may have been additional amongst those that didn’t respond.

Poll 4 –LDCelebrateMe Journey

During the second chat, we asked for participants to tell us if they had joined us in the first chat, this provided an interesting result, as such we decided to run this poll again and add in an option for anyone who had filled in the online survey.  It was fantastic to see that we had such a large percentage of people who had already engaged with the chats previously, it was also good to see the number of participants who had filled in the online survey.  Everyone shared the understanding of the importance of this project, including our colleagues and friends from outside the LD Nursing profession and the responses on this poll echo this sentiment. 

Transcript Review

Initial conversations précised themes of previous chats, the dual activities in clinical practice, of providing and delivering person centred care and support to individuals, while also supporting and facilitating access to mainstream services and community. The role that Nurse Education might play in this was touched on, including the importance of all fields learning together, which might have the dual role of educating other fields of the issues of caring and treating people with learning disabilities while also having a greater awareness of the Learning Disability field.

A parallel discussion linked the positive strategic projects that are currently being led by RNLD’s, including Economic Analysis (RCNI, 2015), the Learning Disability Service Standards (NHS Improvement, 2018), and Learning Disability Benchmarking (NHS Benchmarking Network, 2018) work, with concerns and anxieties about the future of the field based on the recent stories in the nursing media, and UCAS data that indicates that Nursing applications have reduced by 40% in the last two years (Nursing Notes, 2018). An sharing of a Summit – organised by Learning/Intellectual Disability Nursing Academics Network and the UK Learning Disability Consultant Nurse Network being held at the end of July 2018, was an important opportunity our profession to acknowledge the current challenges and develop some creative solutions, and plans to enrol health, social care and education in.

Aligned with this, was the fact that several contributors advised on their own projects, including research, private initiatives, local projects or opportunities abroad. From this we could interpret that we have a number of entreupnurses who are driven by their values and beliefs into creating something new, along with this there are many opportunities for Learning Disability Nurses today, there will likely be more tomorrow and in the future.

Participants indicated that maintaining the energy to represent profession in the face of the stereotyped responses was a challenge for learning disability nurses, even our energetic positive and valued colleagues can take on these stereotypes. 

These sub/un conscious narratives that we might have about ourselves and our profession can be inadvertently activated by those who know no different, and reinforced by those wishing to be positive about the profession. Werner et al (2012) talked of stigma of three main constituents, stereotyping prejudice and discrimination, in this case our profession will often limit itself by its own stigma.

Thanks to Hazel Powell and Ruth Northway for signposting us to this vital publication by Duncan Mitchell (2000) that reflects the parallel history of stigma that people with learning disabilities and Learning Disability Nurses have faced over the last century. Of great interest was in 1926 – just seven years after Mental Deficiency Nursing was acknowledged as a profession- officials were observing that they couldn’t be proper ‘sick’ nurses, as there was no treatment for the disability, which underlines the assumption that all nurses have to be inextricably linked to the medical model, and by consequence the assistant to the doctor in healing.

If this was true in 1926, and has resonances in 2018, we might wish to consider whether we will ever be free of this narrative, and consider other stigmatised groups for inspiration as to how to handle it. How can we hope to provide truly person centred care that empowers, without our own enlightenment to the stories we carry around with us?

Buresh & Gordon’s (2013) book from Silence to Voice, while written about general nursing in the United States, has particular resonances with this conversation. They cite extensive surveys of Nurses, that indicate while there is a belief that the general public have a high degree of trust in the profession, those same nurses go on to state clearly, their frustration that the public do not have a good understanding of their role as nurses and the knowledge skills and experience that is required to be a nurse.

Buresh & Gordon go on to berate the reader and the profession for the infantilising that the profession does to itself, and while some of this may be brutal, it offers the reader a challenge to consider how the profession is presenting itself to the world, asking would other professional groups do the same.

This work provides some vital lessons for RNLD’s, what are the stories that might unconsciously run our individual interactions? How do we enlighten ourselves to these? Which of these do we want to maintain, and which ought we dispense with?

Buresh & Gordon are not nurses, they are journalists with a passion for nursing. They were better able to support and challenge the nurses they worked with, as they did not carry the baggage of the profession. In the Foundation of Nursing Studies, we currently have something similar, expert facilitators, passionate about nursing, who can help us see some of the blind spots that we might have about the profession, similarly to the review and reflection that Mitchell offered. 

Conclusions 

The Foundation of Nursing Studies’ Celebrate Me project has commenced on a journey with Learning Disability Nursing to co-create a new narrative for the field, to achieve this, an enlightenment stage is required as to the old narratives, this has and will offer us opportunity for reflection as to whether these stories are the inspiration we and our successors require for our practice and ultimately the health and wellbeing of people with learning disabilities and their families in UK society.
