On the 30th October we will be joined by guest host Chris Morris from PenCRU for a chat on training for ward staff to improve communication with children who have a learning disability when they are inpatients.

Chris is the academic lead for PenCRU childhood disability research group at the University of Exeter Medical School (www.pencru.org). Formerly he was an orthotist for 20 years. PenCRU (Peninsula Cerebra Research Unit) conducts a broad programme of applied health research to develop and evaluate ways to improve the health and wellbeing of disabled children and their families. They have a reputation for innovative public engagement working in partnership with families with disabled children through their Family Faculty following NHS Englands #expofcare. They aim to produce evidence that users of research consider relevant, useful and accessible, and that inform important real life decisions being made by families, clinicians and commissioners. 

Hospital admissions in children with neuro-disability are common and it is therefore crucial that communication between staff and families is clear. Communication is particularly an issue for children with a learning disability or those who find social interaction difficult. Over several years PenCRU have worked with parent carers, clinicians and researchers to develop and test a training package for health professionals to improve communication with children in hospital. Existing research was reviewed focussing on factors affecting disabled children’s experience of being in hospital. The findings suggested that communication plays a critical role in children’s experience of care. PenCRU also conducted a qualitative study with parents of children that had recently stayed in hospital, and with staff working on children’s wards in Exeter and Barnstaple. The findings identified barriers to effective communication, and also facilitating factors likely to improve communication.

The final training package consisted of information from both studies as well as video clips to prompt reflection and discussion. This can then be replicated and implemented throughout a plethora of hospitals to improve the communication of all ward staff with children who have a learning disability.

PenCRU have also worked in collaboration with students with learning disabilities to create a top tips for communicating document. These include: It’s ok not to know, Just ask, Find out how the child says yes or no and Talk with the child not just the parents.

During this chat we would like to discuss your experiences with children with LD who are inpatients, specifically focussing on communication. We’d also like to know your experience and thoughts around training for ward staff who work with inpatients (children with learning disabilities). This could include what it should consist of, the importance of it and barriers you may have come across and how these can be overcome.

Following the tweet chat held on the 30th October, @WeLDNurses would like to extend a big thank you to Chris Morris from @Pen_CRU for guest hosting his first chat. Here are Chris’ thoughts reflecting on his hosting experience and summarising content brought up throughout the chat:


This was the first tweet chat @Pen_CRU hosted, and was a fabulous experience. An hour goes rapidly in this context! Brilliant to have so many bright people sharing ideas. A few themes that emerged are below. 

I suspect we all share the same views, and our challenge is to spread the word, and more importantly good practice in communication with people who have learning disabilities. We perceived that Learning Disability Nurses could play a key role in such initiatives. 

We at PenCRU would be happy to share the parent-inspired training package we developed over several years – contact details below (or find us on Twitter). Challenges mentioned were ‘same challenges are found across services with both children and adults with LD. Resources, staffing levels, time, training. Staff may also lack confidence in this area due to lack of experience and knowledge’. Suggestions for improving communication and care were hospital and/or communication passports for children with LD, badges, traffic light systems, and making sure staff seek to use them, especially in emergency settings, and also more specialised methods that require training such as PECS Picture Exchange Communication Systems and Makaton. There was also a nice reminder ‘to treat them like children, laugh, keep it simple and get on their level’. 

There was huge support for training ‘all staff’ on communication with children with LD in an inpatient setting, including medical, clinical, clerical and catering/ cleaning staff. The only question was whether it should be mandatory, as some have a negative reaction to training when it is mandatory. There was mention of monthly learning disability awareness sessions, and taking opportunities for including disability awareness training in other study days to solve this issue. The Education Outcomes Framework refers to NHS staff developing values and behaviours, through training, to enhance the quality of the patient experience. Surely a mandate for training to improve communication to improve experience of care? It was highlighted that ‘every person working for the NHS will get training in autism and learning disability awareness’. Though someone commented that awareness wasn’t enough as staff need skills and competence.
