Growing old with Learning Disability - Dementia matters


People are living longer, this includes people with learning disabilities, who are statistically more likely to develop dementia than the rest of the population. People with learning disabilities are also more likely to experience younger onset of dementia symptoms. 

But how does dementia affect people with learning disabilities? The Alzheimer’s Society identifies a key issue is diagnosis, which is often more delayed in people with learning disabilities. This may be due to the person’s inability to articulate their symptoms in a way their carers can understand. Behavioural changes may be misinterpreted as ‘challenging behaviour’ and referred to the CLDT for support. Any assessment of functional skills is insufficient to support a diagnosis when there is no baseline assessment to compare with. Delayed diagnosis, delays in treatment, lack of awareness of how symptoms of dementia can be managed and continued diagnostic overshadowing… 

How can we LD Nurses support the assessment, care planning and support for people with dementia?

Dementia is increasing for everyone, including people with learning disabilities. But as we chatted tonight it became clear to me there are still significant challenges supporting people with learning disabilities to navigate the often complex pathway from first symptoms, through to diagnosis and symptom management. 


It’s so important to be aware of the risk of diagnostic overshadowing. That early symptoms of dementia in people with learning disabilities may be difficult to identify and misinterpreted as behaviour which challenges. This makes the need for baseline cognitive and functional skills assessment vital. 

It’s also always important to consider all potential reversible causes of any change in cognition and functional skills such as infection. 


Baseline assessment, observing behaviour and being aware of any small changes in presentation and what they may mean, adapted assessment and primary care health assessments all featured in the discussion, and were identified as key drivers in improving dementia pathways for people with learning disabilities. 


So once we’ve identified there may be a decline in cognitive function indicative of dementia, we still need formal diagnosis and then treatment and management plans. How do we involve the people we serve and is there enough accessible information about dementia to support people with learning disabilities to make informed decisions about their care and treatment? 

Is there research about efficacy of dementia medication in people with learning disabilities? If there isn’t should there be? And what other therapies can be used? 


Way back when I worked in community services we ran a 12 week course for people with learning disabilities who had dementia. It was designed and run by a speech therapist with input from a music therapist, psychologist as well as learning disability nurses. We did some memory work, themed around topics such as food, holidays, festivals, sang songs, acknowledged fears and emotions and supported some care planning.

Any group work ideas? Adapted assessments? Do any of you have any ideas of what we could do to improve services?

@lynnewalsall Agreed… how do we identify the underlying cause of issues and make sure they are attributed correctly? So important for a baseline to be established and have people that know the individual well involved in any assessments to help measure deterioration #WeLDNs

@WeLDnurses I’ve mentioned in another reply but the value of having the input of people that know the individual the best is immeasurable… high staff turnover is a real issue when completing assessments… so important to have consistency as opinions can be subjective #WeLDNs

@WeLDnurses We’ve used the DSQUID and I worked in a service that used an ‘Oliver and a Crayton’ I think it was called, I liked it because it really involves the person in the assessment where some of the others are more reliant on others perceptions #weldns

Also, depending on how community services are provided - here, Derbyshire Community Healthcare Trust is rated highly for both provision of Learning Disability services (inpatient & community) and Community Nursing inc Older Adults/Dementia. So possibly quite good. #weldns

@WeLDnurses It seems so simple, but so important… make sure to inform the person about risk factors, assessment processes and diagnosis. This must be done with the person and not to them. #WeLDNs
